Crossroads II

24-hour Care

Crossroads Living Center is proud to offer the highest quality, comprehensive, around-the-clock care in two residential living spaces in the town of Dixon. Each facility is licensed by the California Department of Public Health, and is fully staffed and equipped to assist six adults with varying abilities to live as independently as possible in an upscale residential setting. Our services are offered based on individual plans of care, and coordinated and provided by a dedicated team of professionals, which consists of an Administrator, Qualified Intellectual Disabilities Professional (QIDP), Registered Nurse, Manager, Shift Leaders, and Direct Care Staff. Services include assistance with activities of daily living, housekeeping, meal preparation and family-style meal service, medication administration, recreational activities, outings, and transportation.

Person-Centered Planning

Each individual's daily schedule, routine and personal goals are based on their interests, desires and needs, which are established by the individual, with the assistance of their interdisciplinary team. This team consists of Medical Doctors, Physical Therapists, Social Workers, Crossroads Staff, Psychologists, Nutritionists and Families. From there, goals and objectives are developed and carried out to ensure that each person is living their life to the fullest. 

Active Treatment

Our team utilizes a 24-hour continuous, comprehensive, and structured approach to delivering healthcare and other services. It’s not merely about providing care—it’s about empowering individuals to lead fulfilling, self-sufficient lives. Our services consist of a wide range of therapeutic, habitation, and rehabilitation activities designed to help each person achieve the highest level of independence and self-sufficiency.

 

Key components and goals of active treatment:

  • A comprehensive, individualized program that includes services around the clock

  • The integration of consistent training approaches, materials, and learning opportunities throughout each 24-hour period

 

Goals of active treatment include:

  • Helping individuals with developmental disabilities acquire behaviors necessary for self-determination and independence

  • Preventing or decelerating any regression or loss of current optimal functional status, which empowers individuals with developmental disabilities to lead enriched, independent lives
